St. Augustine Home Health Care is a Medicaid certified agency that accepts Medicare, Medicare HMO’s, Medicaid, VA benefits, and other insurances providers.  With St. Augustine Home Health Care you can expect a reliable, rapid response every time.

• Recent discharge from hospital or extended care facility

• Multiple diagnoses or medical complications

• High-risk disorders

• Frequent Hospitalizations or emergency room visits

• Acute episode of chronic illnesses

• Multiple medications requiring instructions/monitoring

• Pain management

• Ongoing management of indwelling catheters

• Wound care

• New diagnosis requiring instruction

• Frequent falls

Services

Your personalized care plan will be tailored to your individual needs and your physician's requirements.  Services are offered to adults 21 years old or older, with a two-hour minimum wherever you call home...we can come to you!

We offer:

• Skilled Nursing Care - Licensed RNs and LPNs

• Physical Therapy

• Occupational Therapy

• Speech Therapy

• Wound Care and Dressing Changes

• Medication Management

• Personal Care

• State Tested Nursing Aides

• Certified Home Health Aides

• Medical Social Worker
   

Medicare eligibility includes:

1. The patient must be a Medicare beneficiary.
2. The patient must have a need for skilled services.
3. The patient must require considerable and taxing effort to leave home or
    temporarily or medically restricted.
4. The patient must meet the criteria for part-time or intermittent services.
5. The patient must be under the care of a physician (MD, DO or Podiatrist).
