To meet the changing needs of those we serve, our continuum of care includes transitional care, rehabilitation, sub-acute care, long-term care, assisted living, and referral assistance for home care and other services.

Each individual at St. Augustine Health Campus is treated with his or her special needs and wants in mind. Our exceptional clinical expertise is a vital element in the overall focus on the physical, spiritual, emotional, and social aspects of our care. 

Personalized patient care plans are developed by registered nurses in conjunction with physicians and other professionals including social workers, dietitians, and on-site occupational, physical and speech therapists.
